Background of the Study: Life quality can be identified through the health of a family and it can also be seen from the health quality of mother and children as it supports the healthy human resource. Public Health Office in Wonosobo showed health profile specifically in 2019 that the number of Maternal Mortality Rate (MMR) was of 126,83 and Infant Mortality Rate (IMR) was of 8, 24. Meanwhile, in Puskesmas Kejajar 1, still in the same year, there were 1 case of MMR with Severe Preeclampsia (PEB) and 16 cases of IMR. Midwives have intention to assist government program in decreasing MMR and IMR by using CoC. Purpose of the Study: To provide comprehensive care for pregnant women during pregnancy period, labor process, newborn baby period, neonatal period, puerperium period and also the employment of family planning program (KB) based on standarized service by applying continuity of care approach and to document it in SOAP format. Method: This research is descriptive one. The subject of the research is Mrs. N 36 years old with high-risk pregnancy and mild anemia. The study was conducted in Puskesmas Kejajar 1 started from 7 February 2020 until 10 May 2020. Result: The pregnancy was in high risk condition related to the age of the mother and she suffered from mild anemia. The delivery process was spontaneous. The period of newborn baby was spontaneous as well and without complications. Period of neonatal and puerperuim were normal. The subject received family planning program (KB) by per-3-months injection. Conclusion: The significant problems appeared during the provision of midwifery care were the risk of older age and mild anemia. Serious complications were not visible on both mother and baby.
